NORTH TORONTO LAWN BOWLING AND CROQUET CLUB – RELEASE AND INDEMNITY AGREEMENT In consideration of membership in the North Toronto Lawn Bowling and Croquet Club (the “Club”) and of entitlement to the use and enjoyment of the Club activities and the facilities licensed from the City of Toronto (the “City”) the undersigned hereby agrees as follows as evidenced by the electronic signature at the end of this document. The undersigned agrees, to the fullest extent permissible by law, that the Club and its directors, officers, members, volunteers, and the City and its representatives, shall not be responsible or liable for any injury or death to persons, personal injury, loss or damage to property damage, expense, loss of income or loss of any other kind suffered by participation during, or as a result of the activities of the Club, caused in any manner whatsoever including, but not limited to, the negligence or intentional acts of the Club and/or City arising or to arise by reason of participation as a member of the Club. To the fullest extent permissible under law, the undersigned agrees to indemnify, defend, protect, and hold harmless the Club, the City and its representatives from and against any and all claims, damages, expenses, judgments, liabilities and costs, including reasonable attorneys’ fees, arising or alleged to arise from any act or omission committed by the undersigned. The undersigned recognizes that participation in the Club’s activities may include risk to health or injury and hereby willingly assumes such health risk or risk of injury, and assume full responsibility before, during and after participation in the activities, in consideration of the acceptance of application and the permission to participate in the activities. The undersigned has read and understood this agreement, and has executed voluntarily, and agrees the agreement shall be binding upon the undersigned, heirs, spouse, children, guardians, next of kin, executors, administrators and legal or personal representatives. The undersigned further acknowledges by signing this agreement he/she has waived any right to maintain a lawsuit against the Club and/or the City on the basis of any claims that are released herein. I hereby agree to abide by the club policies / rules / procedures. I understand and agree that by typing my name below that this shall individually serve as my "Electronic Signature" and that this Electronic Signature shall be deemed to have the same effect as my hand written (or equivalent) signature.
